(KTI) REVIEW OF THE COMPLETENESS OF FILLING OUT THE DISCHARGE PATIENT SUMMARY FORM ON PATIENTS WHO DIED AT PERMATA HOSPITAL CIREBON

The completeness of the discharge summary will greatly affect the quality of
hospital services, where the completeness must be filled in completely by doctors and
health care providers. The purpose of the discharge summary is to maintain the

continuity of medical services of high quality as well as the patient's history when re-
hospitalized, assessment materials for hospital medical staff, fulfilling requests from third

parties or patient health institutions. This study aims to determine the completeness of
filling out the discharge patient summary form for patients who died at Permata Cirebon
Hospital.
The type of research used is descriptive research with a quantitative approach.
The variable in this study is the Completeness of Filling Out the Patient Summary Form
Home in Deceased Patients. The population in this study is the entire summary form of
patients who died at Permata Hospital Cirebon 2022 as many as 520 medical record
documents. The sample in this study was 83,87 or 84 documents of patients who died
using the slovin formula. The sampling technique in this study is random sampling. The
instrument used in this study was a checklist sheet. The research time starts from the
preliminary study conducted on 27 March 2023 and this research is carried out on 17-18
April 2023.
Based on the results of the completeness of filling out the summary of patients
who died at Permata Cirebon Hospital, the patient identification review obtained a
percentage of 97,4%, the review of important patient reports obtained a percentage of
97,8%, the patient authentication review obtained a percentage of 61,5%, and the
documentation review obtained a percentage of 100%.
For the recapitulation of the results of quantitative analysis, the completeness of
filling in the summary of patients who died at Permata Hospital Cirebon was declared
complete, 30 (35,7%) and 54 (64,3%) were declared incomplete. Suggestions for hospital
management to conduct regular meetings and socialize to doctors and other health
workers so that they can complete the patient summary form for the deceased patient
completely and correctly after taking actions and examinations.
