Abstract
Filing out the outpatient registration form for new patients is a form of
documentation for the first time when the patient visits a hospital or health facility. At this
stage the officer will interview the patient regarding the patient's demographic or identity
data. This study aims to determine the completeness of filling out the outpatient registration
form for new patients at the Muhammadiyah General Hospital, Cirebon City.
This type of research uses quantitative descriptive methods. The population of this
study was 580 outpatient registration forms in March 2024. Based on the results of the
Slovin formula calculation technique, a sample size of 85 forms was obtained. Sampling in
this study used simple random sampling. The instruments in this research used observation
and checklist sheets.
The results of this research, from a total sample of 85 forms taken, showed that
there were 39 incomplete outpatient registration forms or a percentage of 46%, while there
were 46 complete forms or a percentage of 54%. With these results, there are still
incomplete filling in the outpatient registration form.
Incomplete filling out of registration forms can be caused by several factors,
including a lack of staff motivation to fill in medical record forms, especially patient
registration forms, and staff's lack of understanding of the importance of filling out medical
record documents completely. Thus, there is great hope that the results of this research can
increase the awareness of officers to complete medical records completely in order to
provide optimal, accurate and efficient services in health services.
