Abstract

Coding is the assignment of codes by using letters and numbers or a
combination of letters and numbers that represent data components. Activities
carried out in coding include coding disease diagnoses and coding medical
actions. he purpose of this study was to determine the accuracy of the stroke code
at the Pantura M. A Sentot Patrol Hospital Indramayu.
The type of research used is descriptive with a quantitative approach. The
population in this study were 284 medical record documents for inpatient stroke
cases in 2021. The number of samples is 74 medical record documents. The
sampling technique used is simple random sampling. The data collection
technique used the observation method with the research instrument used was a
checklist sheet. This research was conducted in the medical record unit at the
Pantura Hospital M.A Sentot Patrol Indramayu.
The results showed that of the 74 medical record documents of stroke case,
51 (68.91%) medical record documents were correct and 23 (31.08%) medical
record documents were incorrect.
The inaccuracy of the code in the diagnosis of stroke cases is due to the
large workload of the coder so that it is less precise in coding the disease. This is
also coupled with the absence of CT-Scan results as a support. It is hoped that the
coding officer can increase the accuracy in determining the code in order to get
the right code.
