Abstract

Accuracy identification patient is a bunch of information or as well as recording data
all information about fact from someone. Patient identification can avoid medical error or
unexpected events for patients. This study aims to determine accuracy identification patient in
writing medical record documents at The Health Center Sumber Kabupaten Cirebon.
This type of research used descriptive quantitative method. The population in this study
is all medical record documents at The Health Center Sumber Kabupaten Cirebon which
amounts to 29.274 medical record documents.The sample used in this research were 202
medical record documents. This research uses accidental sampling techniques. The instrument
of this research is a sheet check list and data were analyzed univariately.
The results of this research was that the accuracy identification patient in writing
medical record documents based on name writing at The Health Center Sumber Kabupaten
Cirebon appropriate as much 178 (88,11%). Writing date of birth appropriate as much 198
(98,01%). Writing medical record numbers appropriate as much 202 (100%). Writing the
patient’s address appropriate as much 200 (99,00%).
Based on results and conclusions in the discussion chapter, the author make the
following suggestion: officers must be more careful in writing medical record documents.
