Abstract

Coding activity is an activity that transforms disease diagnosis, medical procedures and
other health problems from words into a form of code either numeric or alphanumeric to facilitate
storage, retrieval and analysis of data that will be used in statistical reporting. The purpose of
this study was to determine the Level of Accuracy of Disease Codification in Outpatient Morbidity
Reports at the Muhammadiyah General Hospital of Cirebon.
The method used is descriptive with a quantitative approach. The data collection
procedure used is by means of observation and checklist sheets. The population in this study was
the number of outpatient visits for 3 (three) months from January to March 2025 with a total of
864 documents using a random sampling technique using Microsoft Excel and 87 samples were
obtained.
The results of the study obtained from 87 samples of outpatient medical record documents
that have been studied at the Muhammadiyah General Hospital of Cirebon, there are still
inaccurate disease codes. The correct disease code according to ICD-10 is 37 documents with a
percentage of 42.53% while the inaccurate disease code is 50 documents with a percentage of
57.47%.
The inaccuracy of the code is because it is not in accordance with ICD-10 and there are
several codes that do not match the diagnosis so that the writing of the 4th character code is still
not appropriate which causes inaccuracy of the code.
