Abstract

The accuracy of the RME form for this research examination is still incomplete,
specifically the patient's medical history and examination procedures. The diagnosis of the
disease determined by the doctor will greatly influence the patient's treatment and even the
actions taken. Accurate information is based on the history, physical examination, and
supporting examinations, and is recorded in an electronic medical record file/document.
This research used a quantitative descriptive method and design. The population
consisted of all 1,995 medical records from May 2025. The sample size was 100 medical
records. The research instrument used an observation sheet, and data were collected using
primary and secondary data.
Based on the research results, it was found that from the 8 questions asked to the
form, the electronic medical record accuracy of the form from the analysis results obtained
an average percentage of correct electronic medical records of 83% and the number of
incorrect electronic medical record forms was 17%.
The conclusion of this study is that the RME form at the Pabuaran Community
Health Center in Cirebon Regency is not 100% accurate. Suggestions are needed to conduct
outreach to pay more attention to the accuracy of medical record document data so that it
can improve the quality of services at the Pabuaran Community Health Center in Cirebon
Regency and help the implementation of KLPCM after the service is completed to identify
documents that are incorrectly filled out.
