Abstract
Workplace accidents are a critical indicator of the effectiveness of occupational health and
safety (OHS) systems in industrial settings. This study aims to analyze the relationship between
Unsafe Action and the incidence of work accidents in the net-weaving production department at PT.
Arteria Daya Mulia.
A quantitative approach with a cross-sectional design was employed. Data were collected
through questionnaires distributed to 67 workers and analyzed using the Chi-Square test. The results
showed a significant relationship between Unsafe Action and work accidents (p = 0.010). Among
workers with a high level of Unsafe Action, 55.3% had experienced work accidents, compared to
only 24.1% in the low Unsafe Action group.
This finding aligns with Heinrich's theory and James Reason’s Swiss Cheese Model, which
emphasize the dominant role of human behavior in the occurrence of accidents. In the production
floor context, Unsafe Action commonly occurred when workers handled the net-weaving machines
without following safety protocols—such as removing machine guards or performing maintenance
without shutting off the power.
The study concludes that controlling Unsafe Action, strengthening safety culture, and
improving training and supervision are essential strategies for reducing workplace accidents in
manufacturing environments.
